Hands on Living provide a safe and nurturing space for all participants to explore new opportunities and develop lifelong skills. Our team goal is to foster relationships with participants through respecting differences and ensuring the participant is seen and heard. Our program pathways are designed to identify new ways of delivering support services, increase participant motivation, teach lifelong skills, and inspire participants to become a better version of themselves.

New Home Owners

Home and Living trials

Home and Living trials are guided by the Occupational Therapist (OT) who specialises in creating a nurturing experience. Trials allow the OT and support team to foster a relationship with the participant, through trust and respect. The benefits of a home and living trial is to gain a full insight into a participant's ability to manage daily living tasks, level of community engagement, and interactions with other support and service providers. Home and living trials provide the OT a detailed insight into the true support required for a participant.

 

What does this mean for the participant?

  • Choice and control when planning each trial.

  • Opportunities to explore new supports and services, utilising a network of trusted service providers. 

  • An accurate OT report based on evidence, experiences and examples.

  • Unique and appropriate recommendations.

Nurture Program

Our Hands on living team provide a unique support experience.  We not only provide daily and routine support, but we also encourage exploring new opportunities through activities, experiences and recommendations of services. We aim to work with participants, family members and other services to provide the best support plan for participants. We regularly review each support plan to ensure participants are motivated, heard and make necessary changes to encourage progress towards participant goals.

Capacity Building

Our Hands on Living team provide a safe space for participants to develop a growth mindset. We encourage and motivate participants to build on their individual skills, knowledge, and ability to perform daily tasks or activities at a greater capacity.

Building a participant's capacity enables them to become confident, resilient and improves their health and wellbeing.

Capacity building involves the process of trial and error where individuals are encouraged to learn and retain skills and knowledge through practicing tasks and activities in a positive support environment.

Occupational Therapist

The Occupational Therapist (OT) will identify a participant's strengths, difficulties, and help with exploring life solutions. The OT's aim is to encourage and guide participants to successfully improve daily life and aspire to their unique goals. 

Therapy Assistant

Working directly under the supervision of the Occupational Therapist our Therapy Assistants will work towards delivering the best care plan for each participant by enabling participants to make their own informed decisions. They provide specific support to promote, empower and inspire participants to reach their goals.

Support Mentor 

Our Support Mentors provide guidance and support to enable participants to make their own daily choices. Our focus is on delivering support that opens doors to opportunities, as opposed to limiting participants through restrictions and perceived barriers.
